Objective To investigate the value of the Bosniak classification system in diagnosis and management of incidental renal atypical cystic lesions according to CT findings.

目的 探讨依据Bosniak分级标准用CT评价肾脏不典型囊性病变的诊断及处理。

Of the incidental renal atypical cystic lesions, the lesions requiring surgery can be accurately separated from those that can be safely followed up using the Bosniak classification system according to their CT features.

按Bosniak分级标准的CT表现，能准确地将需手术治疗的肾脏不典型囊性病变与仅需随访的病变加以区分。

Results: 43 cystic renal occupying lesions were found in 37 patients, According to ultrasound diagnostic criteria, l3 were hypoechogenic, l7 were isoechongnic and l3 were hyperechogenic. Category of these lesions under CT examination was based on the Bosniak system. 26 lesions of class Ⅱwere found in 24 patients, 12 lesions of class Ⅲin 8 patients and 5 lesions of class Ⅳ in 5 patients . Open operation was done on 18 cases including 4 cases of renal carcinoma.

结果：37例复杂性囊性肾脏，发现肾脏占位病变43个。B超以低回声为主病变13个，等回声病变为主17个，高回声病变为主13个；CT诊断Ⅱ类囊性病变24例（26个），Ⅲ类囊性病变8例（12个病变），Ⅳ类囊性病变5例（5个病变）；手术18例，其中4例为肾脏恶性肿瘤。

Results The main imaging of adult polycystic kidney disease signs: abdominal plain film (45 cases) showed renal shadow was lobulated increase; cystic wall calcification (14 cases); polycystic kidney disease and renal milk of calcium (8 cases); pelvis angiography (61 cases), the performance of the renal pelvis, renal calices separated, distorted, elongated; retroperitoneal inflatable angiography (6 cases): Performance increases of renal kidney shadow Shadow and the relationship between the neighboring organs, its boundaries clear and identified; CT scan (30 cases): the performance of the majority of the size of the renal low-density areas, ranging from thin-walled circular; B-mode ultrasonography (47 cases) showed a circular echo-free renal multiple classes of liquid dark area .

结果 成人多囊肾的主要影像学征象：腹部平片(45 例)显示肾影呈分叶状增大；囊壁钙化(14例)；多囊肾合并肾钙乳(8例)；肾盂造影(61例)，表现为肾盂、肾盏相互分离、变形、拉长；腹膜后充气造影(6例)：表现为肾影增大肾影与邻近器官之间的关系，其界限清晰可辨；CT扫描(30例)：表现为肾内多数大小不等的薄壁圆形低密度区；B型超声(47例)表现为肾内多个类圆形无回声的液性暗区。结论影像学检查有助于提高多囊肾的诊断的准确性。

Results? 6 cases in X-ray film revealed round or oval stones in areatus distribution in the areas of renal papillae and medulla. 3 cases of IVP revealed cystic or hair-brushed dilation. 5cases in B-ultrasound revealed little anechoic area and strong echo light spot in radial distribution with acoustic shadow. Unenhanced CT of 6 cases revealed spotted or areatus stones surrounding minor renal calices with collecting tube system dilation and enhanced CT showed artifacts of stones magnification because of the collection of contrast medium around the stones. Conclusion?

结果 X线平片检查6例，表现为肾乳头、髓质区见簇状排列的圆形、椭圆形致密影；IVP检查3例，显示肾集合管囊状、刷子状扩张；B超检查5例见肾髓质内呈放射状分布的小无回声区和强回声光点，其后方伴有声影；CT检查6例，平扫呈环绕肾小盏分布的点状或簇状钙化灶，集合管囊状扩张，增强后因钙化周围的集合管内造影剂聚集，造成钙化影增大的假象。
