目的 通过对100例胆囊及肝外胆管正常结构尚存在的患者ERCP图像资料进行归纳和分析，探讨胆囊手术术前了解肝外胆道系统形态及变异的重要性。方法选出100例胆囊及肝外胆管正常结构尚存在的患者图像，对其形态、汇合位置进行分析。结果 100例患者图像中肝外胆管显示清晰者97例(97%)，3例因胆总管下端开口松弛导致大量气泡进入胆道，但肝外胆管大体形态显示尚清晰，胆囊及胆囊管显示清晰者87例(87%)，13例(13%)仅部分显示胆囊或胆囊管。结论 ERCP可以清晰显示肝外胆管及胆囊管的形态及变异情况，对于胆囊手术术前了解肝外胆道系统形态及变异有重要价值。 ERCP；胆囊管；肝外胆管

Results: MR demonstrated capsule wall and papillary solid lesions in one case with cystic and solid lesions of hepatobiliary cystadenoma. Poly cystic form tan if different sine in sew case, capsule wall and septation was thick, part of capsule wall was obviously thickened and nodes form, intrahepatic bile duct was dilated, large poly-antrum cystic keen form structure in one case, capsule wall and septation were smooth, part of capsule wall was uniformly thicken, capsule wall, septation, mural nodus form and solid lesions in three cases wee all slightly hyperintense on T2WI and slightly hypointense on T1WI. Part of cyst fluid was hypointense on T2WI the diameter if tumor ranged from 3.4cm to 13.5cm. Part of capsule wall, septation, solid lesions and mural nodus west obviously enhanced in arterial phase in three cases, the extent of enhancement showed a little degrade in port vein phase, balance phase and delay phase, but dense to liver parenchyma at synchronization.

结果：3例肝内胆管囊腺瘤中1例呈囊实性结构，可见囊壁及乳头状实性肿块；1例为多个大小不等的多房囊状结构，囊壁及分隔光滑，囊壁局部明显增厚，有壁结节，周围见轻度的肝内胆管扩张；1例为巨大的多房囊状结构，囊壁及分隔光滑，局部均匀增厚；3例中的囊壁、分隔、壁结节及实性部分均呈T2WI稍高信号、T1WI稍低信号，囊内液性部分均呈T2WI高信号、T1WI低信号；肿瘤瘤体大小径线范围3.4~13.5cm.3例肝内胆管囊腺瘤中囊壁、分隔、实性肿块和壁结节动脉期均明显强化，门脉期、平衡期及延迟期强化程度略减低，强化程度均高于同期肝实质。

Fourteen cases were converted to laparotomy (including 4 cases of type I Mirizzi syndrome, 4 cases of type II which all converted to laparotomy. 2 cases with the location of cystic duct into the common bile duct much lower than normal one, 2 cases with the confluence of cystic duct and cho ledoch locating in the posterior wall of choledoch. 1 case with a public wall shared for about 2 cm between the cystic duct and the choledoch).

结果 全组90例，术中中转开腹14例（包括Mirizzi综合征Ⅰ型4例Ⅱ型4例全部中转；胆囊管汇入胆总管过低2例；胆囊管开口于胆总管后壁2例；胆囊管与胆总管共同一侧壁，并并行约2 cm 1例；右肝管汇入胆囊管1例，开腹术中证实）。

目的：探讨彩色多普勒超声在肾脏囊性病变鉴别诊断中的价值。方法：对临床和病理证实的4例囊性肾癌，8例肾脓肿，2例肾包虫，3例肾结核，30例肾囊肿的二维超声及彩色多普勒超声表现进行了分析，对每例病灶的大小、形态、边界、囊壁回声、囊液回声、囊内分隔及彩色血流的有无、性质进行比较。结果：二维超声和彩色多普勒超声可以反映肾脏囊性病变的结构特征及血流特征。

The appearances of nonvascular lesions were as the following : solitary, huge cystic lesions with wall and septa obvious enhancement were found in malignant enbryonal sarcoma; mixed density mass with necrotic cystic change and solid portion enhanced obviously in carcinosarcoma; intrahepatic huge low density, cystic changes, calcification and margin enhancement were found in leiomyosarcoma.

Results:In this group there were 5 mucinous cystadenoma; 4 serous cystadenoma; 4 teratoma; 2 lager ovarian cyst; 4 subserous leiomyoma of uterine. The massive cystic-solid tumor with irregular cystic wall was seen in mucinous cystadenoma and serous cystadenoma; Teratoma appeared non homogenous density with fat density mass; Subserous leiomyoma of uterine showed large cystic tumor with clear edge, and the size of uterine was normal.
