Large craniectomy may reduce the case fatality. The prolonged coma may increase, while the palinesthesia rate was also increased. 采用大骨瓣开颅减压术治疗重型颅脑损伤，远期病死率较低；早期昏迷发生率虽有所增加，但复苏率明显高于对照组；慢性脑积水、脑穿通畸形发生率增加，但通过采取积极治疗，如早期颅骨成形、脑脊液分流术等可获得较好的疗效，因而远期生活质量较对照组好。

Some CT characteristics of BCSDH were studied first.Then,the risk factors for BCSDH were determined by logistic regression model from the following variates: age,gender,prediagnostic duration,site for injury force,coma experience and CT showing after injury.

By the therapy the time of the patients from unconsciousness to consciousness (4. 94 days) in proctoclysis group was significantly shorter than that (6. 9 days) in the control group. After seven days and fourteenth days therapy the increase in the scores of Glasgow-Pittsburgh coma and the total effective rates in proctoclysis group were superior to those in control group.

In this research, sixty-one patients in unconscious state after hypertensive intracerebral basal ganglia hemorrhage was randomly divided into two groups: The proctoclysis group (31cases) was treated with Tongfuxingshen liquid but the control group (30cases) treated with Tongfuxingshen capsul for seven days. Otherwise, the symptomatic therapy was used simultaneously. The curative effects such as the time of the patients from unconsciousness to consciousness state, the scores of Glasgow- Pittsburgh coma after treated seven days, and the scores of nervous functional defects after treated fourteenth days were observed.

本试验将我院脑病中心1999年9月～2002年5月收入的、符合纳入标准的61例出血部位在基底节区，且伴有意识障碍的高血压脑出血患者，按简单随机分组法，分为31例通腑醒神液直肠滴注治疗组与30例通腑醒神胶囊灌胃对照组，两组病人在性别分布、平均年龄、既往史评分、伴发疾病评分、出血量分布、意识状态分级情况等方面经统计学处理无显著性差异，表明临床资料具有可比性。

Hemorrhagic stroke Sprague Dawley rat model was made by injecting collagenase type Ⅶ and hepathrom stereotaxically, Rats were randomly divided into fake operation group, model group and enema group, take orally group, in three time point (3 days, 5 days, 7 days) we observed the change of cerebral vessels permeability, encephaledema and HSP70 immunohistochemistry, HO-1mRNA hybridization in situ.

结果表明，通腑醒神液直肠滴注较灌胃起效快，能尽快促使昏迷患者神志清醒，意识清醒时间平均为4.94天，而灌胃组为6.9天，经统计学处理有极显著性差异；明显改善患者的昏迷程度，第8天治疗组Glasgow-Pittsburgh昏迷评分值平均为29.39分，对照组为26.60分，经统计学处理有极显著性差异；治疗后14天临床疗效比较，通腑醒神液直肠滴注组总显效为54.84%，总有效率为80.65%，而灌胃组总显效为43.33%，总有效率为66.67%，经统计学处理无差异性，可能为样本量过少有关，尚需进一步增加样本量评估。
