Hepatocellular carcinoma is a common disease in Taiwan. HCC frequently metastasizes to lungs (34-70%), lymph nodes (16-45%), bone (6%) and adrenal glands, but metastasis to the head and neck area is extremely rare. A Medline search of the English literature revealed that head and neck area metastasis from HCC may involved the pituitary gland, nasal septum, paranasal sinus, mandible, zygoma, gingiva, tongue base, palatine tonsils, orbit, thyroid and esophagus.

肝细胞癌是台湾常见的癌症，大部分的肝外转移发生在肺脏(34-70%)、局部淋巴结(16-45%)、骨骼(6%)及肾上腺，转移至头颈部较为罕见，回顾英文文献，曾有转移至脑下垂体、鼻中隔、鼻窦、下颌骨、颧骨、牙龈、舌根、扁桃腺、眼球、甲状腺及食道的病例报告，但不曾有转移至鼻咽部的病例。

The sulfamate moiety in the structure of topiramate is chemically related to known CA inhibitors, and in fact, topiramate inhibits CA activity. To further explore the pharmacological action of topiramate, and clarify the possibility of anti-tumor growth and metastasis of CA inhibitors and its relation with AQP1, we studied the effect of topiramate on adhesion of high metastatic prostate carcinoma to laminin, and established the spontaneous metastatic model of Lewis lung carcinoma to studied the influences of topiramate on tumor growth and metastasis and AQP1 expression in lungs and primary tumors of mice.

为了深入研究托吡酯的药理作用，阐明CA抑制剂和水通道蛋白AQP1相互作用影响肿瘤生长和转移的可能性及其机制，我们研究了托吡酯对高转移的前列腺癌细胞粘附于基质成分层粘连蛋白的影响；以Lewis肺癌自发性转移为模型，考察了托吡酯对肿瘤生长和转移的影响；同时观察了托吡酯对荷瘤小鼠肺组织及原发瘤组织AQP1表达的影响；检测了其对血管新生的作用；利用体外AQP1表达模型，研究了CA抑制剂对AQP1功能的影响；此外，用蛋白质组学的关键技术，通过对模型组与托吡酯治疗组血清蛋白质组的比较分析，来寻找与肿瘤转移相关的蛋白质，探索托吡酯的可能作用靶点。

Background and purpose:In recent years, sleeve resection of bronchus and pulmonary artery has been more extensively applied in treatments of lung cancer patients with poor pulmonary functions. This kind of surgery meets the basic surgical principles of lung cancer being maximum both in resecting the carcinoma of the lungs and preserving pulmonary functions,has a good prospect.
