Methods: 1. Experimental laparoscopic liver resection: We selected pigs as experimental models aiming to approach human beings physiological and anatomical condition. After CO2 pneumoperitoneum was established, the ligaments around liver were dissociated. Livers were cut off by the methods such as clamp dissection, microwave tissue coagulator, ultrasound dissector, Ligsure dissection, hand-assisted, short abdominal incision laparoscopy-assisted, and Endo-GIA switcher. The operation time and bleeding volume were compared between different methods of laparoscopic liver resection. The advantages and limitations of different methods were summarized. 2. Clinical laparoscopic liver resection: 15 cases with tumors in segment V of anterior right lobe or segments II, III of left lateral lobe or segment IV of left internal lobe were selected in this study. The liver diseases included hepatocellular cancer, hepatic cavernous hemangioma and hepatic focal nodular hyperplasia. The ligaments around liver were dissociated after CO2 pneumoperitoneum established. Laparoscopic liver resection were carried out by some of the methods combined such as clamp dissection, microwave tissue coagulator, hand-assisted, short abdominal incision laparoscopy-assisted, ultrasound dissector, rotation and suction dissector.

1、实验性腹腔镜肝切除：以猪为实验动物，旨在接近人体的生理、解剖环境下进行实验，建立操作空间后游离肝脏周围韧带，采用钳夹分离切肝法、微波刀切肝法、超声刀切肝法、Ligsure切肝法、手辅助腹腔镜肝切除法、小切口腹腔镜辅助切肝法、Endo-GIA切肝法等方法实施腹腔镜下肝切除，比较不同切肝方法的手术时间、出血量，总结各种方法的优越性及其局限性。2、临床腹腔镜肝切除：选择肿瘤位于肝右前叶第Ⅴ段、肝左外叶第Ⅱ、Ⅲ段及左内叶第Ⅳ段的15例患者，病种包括原发性肝癌、肝海绵状血管瘤、肝脏局灶性增生，建立气腹后游离肝脏周围韧带，采用钳夹分离断肝、微波刀固化后断肝、手辅助腹腔镜肝切除、小切口腹腔镜辅助肝切除、超声刀断肝、旋吸断肝等多种方法分离切线肝组织，暴露管道结构，再采用高频电凝、钛夹夹闭、缝扎止血、止血纱布覆盖、氩气刀止血、生物胶止血、Endo-GIA处理等多种方法相结合处理肝断面，统计手术完成情况、手术时间、手术出血量、术后并发症及术后住院时间等，评价腹腔镜肝切除的可行性及安全性。

Results: among the 40 cases of patients, 33 cases were early and middle pregnant stage, 7 cases were late pregnant stage. 38 cases had lower abdominal pain, 40 cases had abdominal tenderness and enteron symptom, 33 cases had fever. 25 cases were operated on after diagnosis and all were cured, 1 case occurred incision infection, 1 case occurred abortion. 15 cases received conservative treatment, 12 cases were early and middle pregnancy, 5 cases were cured, 4 cases were improved, 3 cases failed in conservative treatment and operated on surgery, 3 cases occurred abortion, among them, 1 case had incision infection. 3 cases were late pregnant stage, all were operated on, 3 cases occurred premature birth, and 2 cases occurred premature death.

结果：40例患者中，处于妊娠早、中期33例，晚期7例。38例表现为右下腹痛，40例均有腹部压痛及消化道症状，33例伴发热。25例诊断后立即手术者均治愈，术后切口感染1例，流产1例。15例行保守治疗，其中，早、中期妊娠12例，治愈5例，好转4例，保守治疗失败3例症状加重转手术治疗，3例均流产，其中1例合并切口感染；晚期妊娠3例，均症状加重转手术治疗，3例均早产，2例早产儿死亡。

Objective:to investigate the effect of damage control surgeryin the theatment ofacute mesenteric ischemia.methods: a retrospective analysis was made on clinical data of eight cases with ami treated by dcs from june 2006 to december 2007.results: operations were immediately performed in all eight patients with ami after final diagnosis,including six cases treated with different limitation of enterotomy; five with temporary abdominal closure and exteriorization of intestine,three with abdominal closure of venous nutrition transfusion container; seven with two-stage operation in different means.five cases survived or improved three died, one of which died two months after discharge due to short bowel syndrome.conclusion:it is very difficult to treat the ami patients owing to the restriction of technology and equipment.

目的：探讨损伤控制外科在治疗急性肠系膜缺血疾病中的应用。方法：对2006年6月~2007年12月应用dcs理念指导救治的8例急性肠系膜缺血疾病的临床资料进行回顾性分析。结果：对8例急性肠系膜血管缺血疾病均于确诊后立即手术探查，其中6例做了不同范围的肠切除；5例做暂时关腹术加肠外置，3例用静脉营养输液袋关腹；7例进行不同方式的二期手术。本组5例痊愈或好转出院；3例死亡，其中1例为短肠综合征出院2个月后死亡。结论：在治疗急性肠系膜血管缺血疾病病人的治疗中，在许多医院因设备和技术等条件的限制，处理十分困难。
