Methods We analyze the acute pancreatitis cases who were cured by conservation therapy retrospectively. We divided the patients into two groups each one has 40 patients.We give mannitol to one group random and not to another group. We compare the clinical symptoms,physical sign fading time ,the Balthazar grade of CT examination after treatment for three days and by statistic methods.

对经保守治疗好转的急性胰腺炎病例进行回顾性分析，从中随机抽取40例辅助使用甘露醇的病例和40例未使用的病例分成两组，通过对比使用组与未使用组临床症状、体征缓解的时间及治疗3天后CT检查的Balthazar评分，并进行统计学处理。

Methods Fiftythree patients with SAP admitted from 1995 to 1999 was categorized as group 1, and 57 patients admitted from 2000 to 2005 as group 2, were analyzed retrospectively. Ranson′s score, acute physiological and chronic health evaluationⅡ score, Balthazar CT grade, presence or absence of biliary tract obstruction, hypoxia, lung infection, shock, abdomen compartment syndrome, hyperlipemia, pleural effusion were analysed logistically. Mortality following different treatments and incidence rate of complications were also evaluated.

回顾性分析安徽省立医院1995—1999年收治的53例SAP患者(第1组)和2000—2005年收治的57例SAP患者(第2组)临床资料，对患者Ranson评分、急性生理学与慢性健康状况评分Ⅱ、Balthazar CT分级以及是否伴有胆道梗阻、休克、高脂血症、腹腔间隙综合征、低氧血症、肺部感染、胸腔积液等因素进行多因素Logistic回归分析，分析不同治疗方法对并发症发生率和病死率的影响。

Symptoms, physical signs, serum concentration of BUN, Cr, AST, ALT, AMS were observed and compared between two groups. The changes of C-reactive protein and result of bacteria culture, APACHEⅡ grades and Balthazar CT grades, open-belly surgery rate, complications, mortality rate, average hospital stay and costs were compared between these two groups.

比较2组患者的生命体征、临床表现、血糖、氧合指数、肝肾功能、淀粉酶、脂肪酶、C-反应蛋白的变化、细菌培养阳性率、治疗前及治疗后的Balthazar CT评分、APACHE Ⅱ评分、开腹手术率、并发症发生率、死亡率、治愈率、平均住院时间以及费用。

Results摘要: The incidence rates of renal dysfunction, shock, cardiovascular failure and gastrointestinal hemorrhage, the score of acute physiology and chronic health evaluation Ⅱ and the frequencies of pulse and breath in the severe hypoalbuminemia group were all higher than those in the mild hypoalbuminemia group (P＜0.05 or P＜0.01). The differences of incidence rate of hepatic failure and the scores of Ranson and Balthazar CT between these two groups had no statistical significance (P＞0.05). The incidence rate of infection and the mortality in the severe hypoalbuminemia group were higher than those in the mild hypoalbuminemia group (P＜0.01) in the later stage of SAP.

The differences of incidence rate of hepatic failure and the scores of Ranson and Balthazar CT between these two groups had no statistical significance (P＞0.05). The incidence rate of infection and the mortality in the severe hypoalbuminemia group were higher than those in the mild hypoalbuminemia group (P＜0.01) in the later stage of SAP. Conclusion: Hypoalbuminemia in the early stage can accelerate the deterioration in pathophysiology of SAP.
